To register online, select a convenient time for your evaluation, which typically takes 10-15 minutes. After your evaluation, our physician will certify your need for a medical cannabis card and provide you with a form to sign for the Department of Public Health (DPH). Return the notarized form to the US420DOC mailbox as instructed in the email. Once received, we will certify your need to the DPH. Your card should be processed within 10-15 business days.
